*If you are teaching a course at USD-SF and you have approved one of your student(s) (limited to 5 instances per course) who missed an exam/quiz to make-up the exam/quiz, we can help in cases where you might not have time or ability to proctor the student outside of class time; complete the Make-Up Exam Form.
